All application forms are available in PDF format. Download, print, fill and submit to your nearest PLC branch. You can also submit forms through the PLC Online portal or the eDoc system for a fully paperless experience.

Note: Each product application requires a completed Application Form, a Customer Due Diligence (CDD) Form, and a Tax Self-Declaration Form.
