How to Apply

Download All Documents
Download the Application PDF, ESG Compliance Check-list, Company KYC, Declaration of Beneficial Owner, and the individual KYC form for all directors, partners, or office bearers.

Complete the Online E-Form
Fill out the Microsoft E-Form online. This is separate from the printed application and is compulsory for all applicants.

Complete & Sign All Printed Forms
Complete the printed Application, ESG Compliance Check-list, Company KYC, Declaration of Beneficial Owner, and all individual KYC forms. Attach certified copies of all supporting documents.

Submit Before Closing Date
Submit the completed package to PLC Head Office, No. 1161, Maradana Road, Borella, Colombo 08 — or call 011-2631071 / 011-2631075 for guidance.
